Community health centers in the Philadelphia region serve predominantly low-income and historically marginalized populations, with approximately 50–60% of patients enrolled in Medicaid. New federal requirements could mean that eligible individuals will lose coverage because of administrative barriers rather than true ineligibility. Current enrollment support is largely reactive and occurs only after coverage has lapsed. Through its project, the Health Federation of Philadelphia will test whether a proactive, technology-enabled system can help Medicaid recipients maintain health coverage amid new federal work requirements and six-month eligibility redeterminations under H.R. 1. The system will identify at-risk individuals before renewal deadlines and connect them with personalized enrollment assistance. The project will determine whether combining payer data, health center electronic health records, automated outreach, and dedicated enrollment support can significantly reduce preventable Medicaid coverage losses among low-income patients. If successful, the model could be scaled across Pennsylvania health centers, helping as many as 50,000 individuals maintain access to healthcare.
